Update to 2025 Health Risk Assessment Program

Attention Valued Partner,

To support quality improvement, retention, and effectuation initiatives and after a fair market value evaluation, Wellcare will change its administrative payment for D-SNP and C-SNP Health Risk Assessments (HRAs) for effectuated applications.

The administrative payment amount will change to $50 for D-SNP and C-SNP HRAs completed in Ascend, Sunfire or an HRA platform developed within your agency, if applicable, on all Plan Year 2025 (PY2025) applications submitted beginning Oct. 1, 2024.*

Key Highlights 

• A $50 payment is available for 1099 broker and Teledigital partners on Special Needs Plans (D-SNPs and C-SNPs) HRA completions for PY2025.
• Non-SNP HRA completions will not be eligible for payment.
• HRAs may be completed up to 90 days after the application submission date.
• Payment for applications submitted for 2024 effective dates will remain the same.
