We have made updates to the MyHealth Questionnaire (also known as the Health Risk Assessment or HRA survey). These updates make UPMC for Life’s previous version outdated. The outdated version will no longer be accepted after May 31, 2024. That means you will not receive the additional $75 payment if you submit the outdated survey with an enrollment application starting June 1, 2024, and thereafter. Be sure that the survey you are using has “Y0069_241662_C” in the bottom left corner of the last page. These will be the only ones accepted starting June 1, 2024.

You can still receive an additional $75 payment for every Medicare and SNP member when they complete the updated survey during enrollment. For a digital copy of the updated MyHealth Questionnaire, click here. You can also order the updated MyHealth Questionnaire on the  Marketing Storefront site today! Simply log in, navigate to “Enrollment materials” underneath “Browse products” and select your selling region.

If you have any leftover inventory of the previous survey, please destroy immediately.
