Revised Medicare Supplement Application Booklets and Brochures on January 1, 2025
The Medicare Supplement application booklets and brochures have been revised due to the 2025 Medicare Deductible, Co-Insurance and Out-of-Pocket amounts.
Updated brochures, PMA3255 with 2025 amounts have also been added to S3. The 2025 amounts were updated on page 9, 10 or 12 of the Life brochure and page 3 and 7 of the Select brochure. The revision dates did not change.
You will need to go on S3 and order the updated application booklets and brochures.
As a reminder:
- Paper application booklets are only needed if the agent is meeting in person with the applicant and not using the electronic application.
- New functionality will be implemented on 12/30/2024 to allow required compliance documents to be emailed to applicants. Due to this new functionality, eApp Application Booklets will no longer be available on S3. Due to this change, if you need to manually send compliance documents to a customer due to a lack of email address, these are the required documents by product:

These documents may be found on S3. We’ll share more information closer to implementation.
The revised application booklets and brochures can be used once you receive them, as long as the policy effective date is January 1, 2025 or later. Please destroy all existing Medicare Supplement application booklets and brochures with 2024 deductibles on January 1, 2025.
Please note effective 1/1/2025, the following states also have updated rates due to the rate adjustment effective January 1, 2025: AL-AZ-IA-IL-IN-LA-NE-NC-OK-TN-TX-WI. These states cannot begin using the January 1, 2025 booklets until January 1, 2025 because of the new rates.
