Plan Highlights for 2025
Flex Card benefits: All plans feature a Flex Card for additional spending. Members in our HMO and PPO plans can use the Flex Card on additional dental, hearing, and vision services; members of our DSNP plans can use the Flex Card for food and utilities.
Lower copays for urgent care: We've lowered copays for urgent care for all plans.
Pennsylvania
Complete (HMO): Our top-selling $0 premium plan in Pennsylvania; Complete maintains strong medical and supplemental benefits.
Giveback (HMO): Featuring a $125 Part B rebate, Giveback has a $0 premium with solid core medical benefits
Flex Pro (PPO): Flex Pro is our new $20 premium PPO plan with in- and out-of-network parity on many medical services.
New Jersey
Choice (PPO): One of our new PPO plans, Choice features a $0 premium and strong core benefits with in- and out-of-network parity on many medical services.
Silver (HMO): Our $0 premium HMO plan in NJ; Silver is one of our flagship plans and features solid medical benefits.
Submitting an Application
Jefferson Health Plans uses Connecture as our online enrollment site. Both plan years 2024 and 2025 are available via our new dual plan year site.
