LARGE GROUP QUOTE REQUIREMENTS
As we head into the thick of this busy season, we wanted to share the most current Quote Request Form (QRF) for each region, an up-to-date map of Highmark's service area, census template, along with a copy of the webinar held earlier this year in March.
Census Template required in Central PA and West Virginia, preferred format for all other regions.

Note – the QRFs vary by region based on the standard plan designs, product offerings, and underwriting requirements.
Pennsylvania
Market segment is determined by the average number of employees on all business days during the preceding calendar year. An employer with greater than 50 total average number of employees is considered large group.
