LifeScan Utilization Management and Formulary Deletion

What do I need to know?

LifeScan, the manufacturer of the OneTouch diabetic supply product line, has declared chapter 11 bankruptcy. In response, Highmark members who utilize diabetic supply products will experience changes.
Effective beginning September 15, 2025, OneTouch diabetic test strips and meters will be removed from the Commercial and Exchange formularies and Preferred Drug Lists, and existing users of OneTouch products will be impacted by the utilization management (UM) changes that follow:
Highmark’s UM policies will list OneTouch products as non-preferred and require a trial of two (2) plan-preferred alternatives. This change is effective immediately for members who are new to therapy.
