■ Members will receive a prepaid Visa card that is loaded on the 1st of every month

Here are the TOP 10 most qualifying conditions on our non-SNP plans:

■ Arthritis
■ Asthma
■ Chronic kidney disease (CKD)
■ Chronic obstructive pulmonary disease (COPD)
■ Depression
■ Diabetes
■ High blood pressure (hypertension)
■ High cholesterol
■ Osteoporosis (weak bones) or family history of osteoporosis
■ Severe obesity (body mass index of 40 or higher)
