Group Administrator's Manual
The Capital Blue Cross Group Administrator’s Manual helps group leaders administer their healthcare benefit programs. The manual offers guidance on enrollment, billing, ID cards, obtaining services, and additional resources, including contacts and frequently used forms.
The manual is subject to change as revisions to Capital Blue Cross contracts, policies, and/or procedures occur. The newest version will always be available on our website, which requires secure login for access. If someone’s responsibilities as Group Administrator change, please notify Capital to request access for the new Group Administrator.
Duplicate or corrected 1095-B tax document requests
Capital mailed the required 1095-B tax document to:
All Fully-Insured Group subscribers.
Off-exchange individual subscribers.
On-exchange catastrophic subscribers.
PersonalBlue subscribers.
Small Group ASO subscribers (Small Group ASO are those groups with 50 or fewer employees).
Should a subscriber require a duplicate/replacement Form 1095-B they should log in to their secure account and can view and print their form under Tax Forms > Manage My Information. Subscribers can also contact Member Services by calling the phone number on the back of their ID card.
If the member identifies a need for an update or correction to the information found on the 1095-B, they should contact their employer to follow established processes that provides Capital with updated information by March 3, 2025. If the member is enrolled in an individual account product that was not purchased on the Pennsylvania State Exchange (PENNIE), an updated enrollment application should be submitted to Capital Blue Cross by March 3, 2025.
1095-B Forms are only issued to subscribers. If the subscriber is enrolled through a group, the group does not receive copies of their employees’ 1095-B forms. Groups do not receive any listings, reports, or any other documentation specific to the 1095-B forms issued to their employees.

Employer group customer invoices are available online. Customers can view and pay their invoice using our employer toolkit.
Capital Blue Cross offers customers simple and secure options to pay their invoices so that they may choose what works best for them.
Electronic check — Groups pay their invoice online by making one-time or recurring payments. This is controlled by the group.
Bill payer — Works with most banks and credit unions to receive payments through a remittance network. Groups must be enrolled in their bank’s program.
Group Check It Out — Available to fully insured and small group ASO customers. Capital deducts payments from the group’s bank account when the group completes an enrollment form on our website.
Corporate Check It Out — Automated payment option for ASO groups to deduct monthly administrative and claim fees from the group’s bank account. Groups need to complete a form to enroll. If the group has multiple subgroups, a form is needed for each subgroup that uses different banking information. An ASO large group (100+) paperwork checklist is available.
Payment by check — Payment should include the check with the group/subgroup ID and the invoice number on the check, as well as the return payment portion of the statement. Do not send other correspondence with the payment to the remittance box.

Capital’s group customers are reminded to pay the premiums as reflected on the invoice. Please encourage them not to calculate adjustments for pending benefit or enrollment changes that were still in process at the time of invoice generation. Applicable adjustments will be reflected on a future invoice.
