
Help eligible members upgrade to C-SNP this AEP
We’re excited to offer new Chronic Condition Special Needs Plans or C-SNPs in several counties in 2025. Aetna Medicare C-SNPs are designed for those with diabetes, chronic heart failure, or four other cardiovascular disorders. These plans provide specialized coverage, benefits and support for those who have these conditions.
Keeping you informed: If members are eligible for C-SNP, they may get a letter from us

We’ve identified some Aetna Medicare members that may be better served by a C-SNP in 2025. We’re sending them a letter about this opportunity.
The letter encourages members to contact their agent for assistance first, but also provides a phone number for Aetna Telesales as an alternative
How you can help
- You can assist these clients with enrolling in C-SNP if they contact you.
- Be sure to help clients complete all C-SNP specific elements of the application, including the Prequalification Assessment Tool (PQAT) form.
- You must specify the eligible chronic condition and contact info for at least one provider who can verify the eligible diagnosis. Applications without this information will be delayed. View FAQs for details.
- Or, if members choose to call Member Services or Aetna Telesales, we’ll assist them. Since plan changes from an MAPD to a C-SNP are considered a “like plan change,” you will remain the Agent of Record (AOR). Review our AOR retention policy.
