Capital’s utilization-based admin fee** funding arrangement provides flexibility to employers interested in reducing or eliminating the traditional per contract per month (PCPM) admin fee for medical claims. Groups that choose this funding option will have a reduced or eliminated PCPM, and a per-claim charge that is based on a review of total administrative fees and historical claim counts.
Benefits of the utilization-based admin fee funding arrangement:
Reduced or eliminated fixed up-front administrative fees.
No impact to the group’s chosen benefit plan design.
No effect on stop loss.
No impact on your employees’ experience with their health plan.
Contact your Account Executive today to discuss if this new funding option is right for your groups.
* Available to large, self-funded customers only.
** Utilization Based Admin Fee, while based on claim counts, is not billed as a claims expense, it is identified and billed as an administrative fee, separate from a group’s claims expense.
When using the non-large group census template, it is important to be sure you're using the most up-to-date version. Look for “This version updated as of 10/1/2025” in the header of the instruction tab to confirm you're using the current version.
Producers completing the census template and submitting the member-level file from the group should keep these points in mind:
Follow the detailed instructions tab to guide you in completing the template accurately thus minimizing processing delays for your groups.
It is extremely important that this information be accurate and based on the anticipated actual enrollment of the group.
Refer to the group’s UC-2 when verifying census accuracy.
Any changes/corrections in the final submission census compared to the prior initial proposal census or risk assessment rate request (enrollment, spelling of the name, residential zip codes, contract type, gender, date of birth, etc.) may result in a revised risk assessment score. Those changes could affect monthly rates for medical and Rx plans.
Capital submitted the Gag Clause Prohibition Compliance Attestation (GCPCA) to CMS on behalf of itself, fully-insured customers, small business ASO group customers, and self-funded ASO group customers who opted in via our webform on December 16, 2025, ahead of the December 31 deadline. Self-funded ASO group customers who did not opt-in will need to submit their own GCPCA to CMS. Consult your legal counsel for guidance.
