Minimum National Order: There must be a minimum of 100 Lead Card Collection Stations requested between all pre-orders received across the country, otherwise we will not move forward with the order. Your credit card will not be charged if the number of total Lead Card Collection Stations ordered does not reach 100.
Operational and compliance guidelines - Obtain permission from venue prior to placing station
- Rules pertaining to marketing materials in provider locations apply; for example, the station cannot be placed where consumers receive care or wait to receive care
- Must use approved lead cards and marketing materials from Agent Toolkit
- If used in provider offices, providers must not steer consumers to the lead boxes OR handle the lead cards in any manner (ex: empty the lead box, forward leads to the agent)
- Information provided on lead cards must be considered private
- Lead collection box must be checked and emptied no less than once a week
- Any suspected or known breach or theft of the lead box must be reported to UnitedHealthcare immediately
