How to Submit Individual Business
ON Marketplace:
- Obtain a quote from your URL Case Manager.
- Use the Kaiser Family Foundation to obtain an estimate of the Advance Premium Tax Credit (Premium Subsidy) that is available to your client.
- Have your client complete the MARKETPLACE application.

- Have your client complete a phone interview with the Health Insurance Marketplace at 1-800-318-2596.
OFF Marketplace:
- Obtain a quote from your URL Case Manager.
- Complete the carrier specific application.
- Fax/email the application to your URL Case Manager.

How to Submit Group Business
ON Marketplace:
- Obtain a quote from your URL Case Manager.
- Still waiting for details from the carriers – SHOP won’t open up until 12/1/13.
OFF Marketplace:
- Obtain a quote from your URL Case Manager.
- Submit carrier specific application to your URL Case Manger.
