How to Submit Individual Business

ON Marketplace:

1. Obtain a quote from your URL Case Manager.
2. Use the Kaiser Family Foundation to obtain an estimate of the Advance Premium Tax Credit (Premium Subsidy) that is available to your client.
3. Have your client complete the MARKETPLACE application.

4. Have your client complete a phone interview with the Health Insurance Marketplace at 1-800-318-2596.

OFF Marketplace:

1. Obtain a quote from your URL Case Manager.
2. Complete the carrier specific application.
3. Fax/email the application to your URL Case Manager.

How to Submit Group Business

ON Marketplace:

1. Obtain a quote from your URL Case Manager.
2. Still waiting for details from the carriers – SHOP won’t open up until 12/1/13.

OFF Marketplace:

1. Obtain a quote from your URL Case Manager.
2. Submit carrier specific application to your URL Case Manger.
