Formulary and Diabetic Supplies Information and Tips
- Macular degeneration/ Part B drug: If you come across someone getting injections in their eyes for Macular Degeneration, the organization Good Days often covers the 20% coinsurance that applies to Part B drugs (which in this case are the eye injections). This can be very costly but Good Days help people on Medicare that are under 500% of the Federal Poverty guidelines. It’s very easy to apply. Don’t make any promises on benefits, but do help a person apply. You will gain the best referral source by helping them. https://www.mygooddays.org/patients/diseases-covered/macular-disease
- Medication NOT on our Formulary? If a medication is not on our formulary, don’t worry, we have a form that the provider can request a formulary exception. Which I have attached it to this email. It would be beneficial for you to print these and carry them with you.
- Diabetics: ALWAYS ask to see their glucose monitor.
- There are two types of monitors. A regular diabetic monitor and a continuous glucose monitor. Most people use the regular glucose monitors. The continuous monitors attach to the arm. We cover both types, however, there are certain guidelines.
- Regular diabetic monitors: We only cover the One Touch Ultra. Use this website to order a monitor. www.OneTouch.orderpoints.com Use Code 123AET200. (Right now they are sending vouchers to use at the store for a free monitor, instead of the actual monitor).
