We’ve recently made some updates to our Health Risk Assessment (HRA) survey. Some of the survey questions have been changed to match other forms and questionnaires that we provide, and we’ve added the Medicare Beneficiary ID (MBI #) as required field to be completed on the form. Please make sure to use this new HRA form when you work with a client to complete this form. Remember! UPMC for Life agents receive an additional $75 payment for every new Medicare and SNP member when they complete a Health Risk Assessment (HRA) survey during enrollment. The Health Risk Assessment (HRA) survey helps our members receive the highest quality of care from UPMC for Life.
