Off-exchange Enrollment:
- The Producer Portal will accept 1/1 enrollment until midnight on December 15th. Paper applications may also be e-mailed to dp_applications@highmark.com or faxed to 866-224-5403. Applications submitted between December 16th and January 15th will be eligible for a 2/1 effective date.
- 2022 SEP forms and applications are available on the Producer Portal, producer.highmark.com, under "Resources" and on Litmos under "Additional References." Please note that SEP effective dates will be determined by the qualifying event indicated on the application.
Producer Verification Form: This form has been revised and will be available for download on the Producer Portal and Litmos next week. Effective immediately, forms should be e-mailed to ACASalesSupport@highmark.com.
