• If you choose to submit applications via fax, please only fax one application at a time. Do not combine multiple applications into a single transmission.
• Applications must be faxed within 48 hours of receipt. 
• Highmark Senior Markets strongly suggests you submit applications through the Producer Portal instead of via fax. We have limited resources in the office this year, and we may see processing delays of faxed applications.
