TIPS FOR FASTER APPLICATION PROCESSING

Recently, we have noticed some frequently missed questions on our Final Expense and Fully Underwritten applications. To help expedite the application process on these products, please ensure that the following questions are completed:

Final Expense Applications:
|
|
1) Page 1 / Part 3: Applicant's Place of Birth. Please include city AND state and/or country.
|
|
2) Page 2 / Part A / Question #2 regarding existing life contracts: Please check yes or no and provide information in boxes that follow question.
|
|
3) Questions #1 and #2 on Replacement Form (This form is located near end of application packet): Please check yes or no for each question.
|
|
4) Page 4 / Part D : Current Medications. **Even if there are no medications to be listed, please write "NONE" in the box below the question.**

|
Fully Underwritten Applications:
|
|
1) Page 1 / Section 3: Applicant's Place of Birth. Please include city AND state and/or country.
|
|
2) Page 3 / Section 12: Replacement and Other Insurance. If the applicant does not have any other insurance in force, please write "NONE" in the box.
|
|
3) Page 6 / Section 14 / Questions #14 and #15: Please check yes or no for each of these questions. If answer is "yes" on either question, please explain in detail in boxes provided below questions on the application. **It is also very important to list any medications that are prescribed and reason for prescription in the boxed section.**
