Health Partners Medicare is now offering a $60 incentive payment to all brokers for completing an Health Risk Assessment (HRA) during a new enrollment in our Special (SNP HMO) plan.

To earn the incentive, you must complete an electronic HRA (fillable PDF) during the enrollment process and submit the completed HRA within 48 hours of the enrollment. The incentive is only available for writing new applications for our Special plan; Prime and Complete enrollments are not eligible for this incentive payment.
