*This email is intended for producers in DE, PA, and WV FORMFIRE UPDATES AND USER GUIDE In 2020 Highmark announced a partnership with FormFire for the purpose of collecting medical information for medical underwriting used for our Balanced Funded product and in Central PA middle market cases. FormFire allows eligible employees of prospective clients to complete a "health interview" on FormFire’s electronic platform. Responses for those employees are then mapped to Highmark’s Medical Health Questionnaire (MHQ).
To further support the use of FormFire for collecting medical information, we’ve developed a user guide to assist in navigating the system. Access this guide here.

The following changes were made in late 2020 to make use easier as well:
- Reduced # of days for Broker Transfers – now 2 days, if the group initiates the transfer.
- Updated Welcome Email for the HR Contact – new format with clear links for creating the HR Manager Account, accessing the Employee Welcome letter, and a video on how HR Managers can effectively use FormFire.
- Updated Employee Welcome letter – with a quick link to get started and a video on how to create an account. Also in the letter are links to FormFire's privacy policy, security info and other helpful videos.
- Lastly, when Highmark medical applications are downloaded from FormFire an excel spreadsheet will also download automatically. The “Highmark_MUW_Census” should be sent to Highmark along with the medical applications. To ensure this spreadsheet is automatically downloaded be sure to use the “send forms”
