Off-exchange paper enrollments must include both the off-exchange application and SEP form. The SEP form has been revised to include an option for the COVID Enrollment Period. These materials are available on the Producer Portal, producer.highmark.com, under Resources. If submitting enrollment via paper, SEP forms are required starting 3/1/21 for the COVID-19 SEP. Completed applications and SEP forms can be e-mailed to DP_applications@highmark.com or faxed to 866-224-5403.
Due to the COVID-19 SEP, off-exchange enrollment will be accepted through the Producer Portal until May 15th.
