The fastest and most accurate application method is using the Ascend Enrollment Platform.

How to Login:

  • Your username is: [email protected]
  • Password: If you forgot your password or need to establish a new password, select the Forgot Password option. You will be prompted to change your password upon first login

Telephonic Scope of Appointment (SOA)

The Telephonic Scope of Appointment (SOA) allows brokers to record and save a compliant SOA per guidelines established by the Centers for Medicare & Medicaid Services (CMS). This option telephonically records the SOA via a phone call through a live agent speaking with the beneficiary.

Telephonic SOA is available for all Wellcare enrollments, including enrollments for Wellcare plans offered by Allwell and Health Net.

An appointment/recording ID will be provided for use on agent-assisted online applications or paper applications will be provided. Be sure to record this number on the application, whether electronic or paper.

 

Personalized URL (PURL):

The PURL is a non-agent-assisted, online application tool that beneficiaries can use to self-enroll and you, the broker, still receive credit for the sale! Every broker is assigned a PURL using your National Producer Number (NPN). The PURL format is www.wellcarerep.com/NPN.

​After PURL application submission and processing, the broker will receive credit for the enrollment. Commissions will reflect on your Centene Workbench Commission Statement, accessible through Single Sign-On.

Note: Brokers cannot complete the PURL application on behalf of the beneficiary or assist the beneficiary with the completion of the PURL application in any way.

 

Paper Applications

Brokers/Agents have the ability to submit paper applications via fax*. To ensure timely and accurate processing, be sure to submit enrollment applications to the correct health plan fax number. When submitting enrollment applications, be sure to use the appropriate broker identifier.

*Note: Wellcare By Fidelis Care: paper applications submitted should include the Wellcare By Fidelis Care unique broker identifier: FB + NPN (ex. FB123456).

Important Reminders

  • Be sure to use the appropriate Application and Scope of Appointment (SOA), determined by the health plan of choice and the effective date/plan year. For example, a 11/1/2023 effective enrollment should be completed on a 2023 application. A 1/1/2024 effective enrollment should be completed on a 2024 application..
  • Enrollment applications must be received by Wellcare no later than the calendar day following receipt from the beneficiary.
 

Paper Application Tools

Personalized Paper Application Confirmation Page:

The Personalized Paper Application Confirmation page for faxed applications includes a barcode that will connect the application to your record, and confirmation of receipt will be emailed directly to you at the email address on file.

Key Points to Know:

  • If contracted through Centene Workbench, the barcode links to your NPN.  Note: It is non-compliant to market/sell if you are not actively certified with the Plan.
  • The Personalized Paper Application Confirmation Page must be submitted as the final page of each individual paper application submission.
  • WARNING! - Only submit one application per fax
  • Within five minutes of the completed fax transmission, an auto-generated email will be sent directly to you at the email address on file.

Use Centene Workbench to Upload an Application:

Brokers have the ability to upload paper application PDF files by uploading applications within Centene Workbench, accessible through Single Sign-On.
