Applications:
If you're enrolling members through the Producer Portal, the online enrollment application is now updated to reflect 2020 rates and plan benefits.
The Central and Northeast Pennsylvania application has been updated to include Community Blue Medicare PPO Distinct as a plan option. Click here to view and download the new application. The original application will still be accepted; however, if Community Blue Medicare PPO Distinct is the desired plan option in Central Pennsylvania, select the Distinct product under the Community Blue Medicare Plus PPO heading, cross out Plus, enter the premium amount, and initial. Click here to see an example. |
