July 2019 Your Choice formulary updates

The Your Choice formulary changes for July 1, 2019 are now available. Please view the attached chart for more detailed information. UPMC looks for ways to maximize the value of its benefits, which sometimes involves adding or removing medications to the formulary. All formulary changes are made with the help of local doctors and pharmacists.

For a complete list of changes, please reference the attached table. Listed below are the key points of the update:

The following generic medication will move from the first-tier to the third-tier:
- Flunisolide nasal spray

The following medication will move to non-covered status. Please review the attached chart for formulary alternatives.
- Besivance eye drop

The following changes for erectile dysfunction (ED) medications applies to clients with this benefit only.
- The following generic medications will move from the first-tier to the third-tier:
  - Tadalafil (generic Cialis)
  - Vardenafil (generic Levitra)
  - Vardenafil ODT (generic Stendra)
- The lower tier formulary alternative is sildenafil (generic Viagra).

UPMC Health Plan will contact members who are using any of these drugs to explain the changes.
