Four easy steps to apply

  1. Critical illness application must be submitted within six months of the preferred life exam.
  2. Applicant must have qualified for preferred life with an approved company (see list at right).
  3. Complete page 1 of Assurity’s application, plus the General Section, Physician Page, Critical Illness Product Section and Critical Illness Health Section.
  4. Send completed application with a copy of the medical exam and schedule page from the preferred life policy.
