include the following information:
- Which statement
you are calling about (month and which statement, if
applicable)
- Member’s name
- Member number
- Effective date of
the plan
- The specific
issue you are calling about
- If you are
calling about more than one member, we will need the name,
number, and effective date of the plan for each of the
members you are calling about.
We will investigate the issue and get back to you
with the information or resolution as quickly as possible.
