CENTRE COUNTY ZIP CODES UPDATED ON INDIVIDUAL ENROLLMENT APPLICATIONS

The Western and Central Pennsylvania Individual Enrollment Applications distributed in October have been updated with Centre County zip code changes. The updated 2019 Individual/Family Plan Health Insurance Enrollment Applications are now available on the Producer Portal under the Resources section.

Please begin to use these updated forms immediately.

  • Page 6: Added all WPA region zip codes in Centre County (not previously listed).
  • Page 11: Removed zip codes 16865 and 16877 from the Conversion/HIPAA products offered in Centre County.

CPA Application

  • Page 6: Added zip codes 16865 and 16877 to the listing for Centre County.
  • Page 11: Added zip codes 16865 and 16877 to the Conversion/HIPAA products offered in Centre County.

Members Enrolled Using Zip Codes 16865 and 16877

Members from these zip codes that were previously enrolled have been contacted and will be moved to the appropriate regional product. No further action for enrolled members is necessary.
