In the event a submitted application triggers a Request for Information (RFI) due to an invalid HICN, agents and/or membersdo not need to call the RFI line to correct that issue. Instead, agents can assist with resolving the issue by completing and submitting the Invalid HICN Correction Form and submitting on the member’s behalf.
Important Information!
- More than one member with an outstanding HICN related RFI can be added to the correction form and submitted in one fax
- If hand written, ensure to write in all caps and block letters
- Separate sheets need to be completed per each product line – CCP and PDP
Once completed, fax to the following numbers per product line:
- 1-866-388-1521 – PDP
- 1-866-473-9124 – CCP
Members with outstanding HICN related RFI’s will receive a notification via mail prompting them to contact WellCare with specific information. Please reference the example letter attached.
