When completing a Prescription Drug paper enrollment application with your client it is important to complete the information on page 4 under the header – ‘For applicants receiving assistance from a broker or agent.
Some points of clarification:
- Provide your National Production Number (NPN) under Broker/Agent ID#. Do not enter your Mutual of Omaha production number.
- The information under ‘Agency’ may be left blank.
- The ‘Carrier #’ is associated with the Rx plan your client is enrolling in.
- Mutual RX Value – 8481
- Mutual RX Plus – 8485
