Ordering 2019 Sales Materials
Producer Services has begun taking requests for sales tool kit materials via Agent Connect Sales Support Tickets. To submit an order, please create a new Sales Support Ticket with SUPPLY ORDER in the subject line. Please be aware you will need to provide the following information:
- First name
- Last name
- Producer ID (PID)
- Shipping address
- Line of business (MAPD or PDP)
- Plan name
- Plan code
- County name
- Supply amount
- Contract ID
If you previously submitted a Supply Order via an Agent Connect ticket, please review the ticket to ensure additional information hasn’t been requested.
Note: 2019 enrollment applications may also be obtained via the left menu on your Agent Connect portal under Enrollment Materials.
