2019 Electronic Sales Materials

Please use the following sources to obtain electronic copies of sales materials you received in your 2019 Sales Tool Kit Package:

• Agent Connect: Download from either the Sales Presentations and/or Enrollment Materials tab of the portal.
• WellCare website: Reference the Find my Plan tool for access to enrollment materials only. Please download the Find my Plan Tool Guide for steps on utilizing the tool.
• District Sales Manager (DSM): Schedule a time for pick-up with your designated DSM.

Please download copies of 2019 enrollment materials applicable to your line of business and/or market:

Enrollment Applications:

• Coordinated Care Plans
• Prescription Drug Plans
• Private Fee for Service
• Employer Group Enrollment Application
• NJ Liberty

Ordering 2019 Sales Materials

Producer Services has begun taking requests for sales tool kit materials via Agent Connect Sales Support Tickets. To submit an order, please create a new Sales Support Ticket with SUPPLY ORDER in the subject line. Please be aware you will need to provide the following information:

• First name
• Last name
• Producer ID (PID)
• Shipping address
• Line of business (MAPD or PDP)
• Plan name
• Plan code
• County name
• Supply amount
• Contract ID

If you previously submitted a Supply Order via an Agent Connect ticket, please review the ticket to ensure additional information hasn’t been requested.

Note: 2019 enrollment applications may also be obtained via the left menu on your Agent Connect portal under Enrollment Materials.
