A Power of Attorney (POA) is legal representation authorized through a document indicating who can make medical decisions on a beneficiary’s behalf. Beneficiaries who do not have the capability to administer their own healthcare plan often elect a POA.

 

In order to add a POA to a beneficiary’s WellCare plan, the following form must be completed and returned to WellCare by fax at 813- 464-8413 or mail at P.O. Box 31370, Tampa, FL, 33631.

Please note the form is initially provided to the member in their Welcome Kit and is also available on the WellCare website under the Find my Plan tool.

Extra Help Medicare Program (Medicaid)

 

 

Medicare beneficiaries with a limited income may qualify for the Extra Help government program and receive state assistance to pay their Medicare Part D prescription drug costs. The program provides support by paying Part D monthly premiums, deductibles, coinsurance and copayments. Eligibility for Extra Help is determined per Medicare Savings Program (MSP) level, and WellCare utilizes MSP levels to determine plan qualification. Please download the following chart for additional information on Extra Help eligibility.
