Quoting Mid-Market Groups with Fewer than 51 Enrollees

Beginning August 1, all new mid-market groups with fewer than 51 enrolled subscribers must complete a risk assessment.

When submitting rate requests for these groups, please include a member-level census along with the group application. Required member-level information includes: first and last name, gender, contract type, date of birth, employment status, ZIP code, and Medicare eligibility. The census must include information for all members—not just subscribers.

Additionally, there has been a change made to our vision requirements chart. The last bullet under Stand-Alone (Vision Only) currently states “Not eligible for Voluntary Vision Product.” This will change to read “Not eligible for Voluntary or any of the Discount Vision programs (Vision Value).” We will share an updated chart soon.

New Paperwork Submission Checklists

We are excited to announce that we will soon offer electronic enrollment! In the meantime, we have revised and updated paperwork submission checklists—now organized by market segment. All enrollment paperwork should now be submitted using the below attachments. Older checklists will no longer be accepted, effective July 1.

Discontinuing Calendar Year Benefits

Effective October 1, Capital BlueCross will no longer offer a calendar year benefit plan for new business quoting; new business will only receive quotes for plan year benefit plans. Stay tuned for more detailed information in the coming months.

Healthy Rewards Reminders

  1. In order to receive a reward card, non-Medicare members must activate their Healthy Rewards program from the Healthy Rewards dashboard, available on their secure member page at capbluecross.com. This step walks members through necessary compliance and terms and conditions language, which they must agree to in order to participate and earn incentives.
  2. If members are receiving a health screening at their provider's office or at a Capital Blue health and wellness center, they must complete the physician/care guide screening form located on their Healthy Rewards dashboard in order to earn credit for it. This form contains a member-specific scan code that allows Bravo to identify their results. A generic form cannot be processed.
