Step 1: Please complete Contract Amendment

If you have previously submitted an Amendment, just notify us in writing via email that you would like us to add Pennsylvania- Midwest

Step 2: Please include authorization to draft the Appointment Fee

Appointment Fee waived if you have submitted business within the last 12 Months

Step 3: Return Amendment & Pre-Authorized Draft Form to our office
