1095 TAX FORMS FOR 2016 PLAN YEAR


Highmark would like to remind agents that 1095 tax forms will be issued to customers from the following sources:

  • 1095-A: Customers who purchased individual insurance from a public exchange will receive a 1095-A tax form in the mail from the exchange.
     
  • 1095-B:  Highmark will mail 1095-B tax forms for the applicable tax year to the contract holder for all Highmark Fully Insured Commercial Groups and Individual Market (Direct Pay) Off-Exchange. This form will show which months each member in the main contract holder's family was insured in minimum essential coverage (MEC) from Highmark.  
    • Members may receive multiple forms, as a different form will be mailed for each group number in which the customer was enrolled.
    • Forms will be mailed in late February.
       
  • 1095-B or 1095C:  If the member was enrolled in a self-funded (ASO, ASC or Cost Plus) health benefits plan, the plan sponsor will mail the member a 1095-B or 1095-C tax form.
