REVISED INDIVIDUAL BLUE EDGE DENTAL APPLICATIONS AND BROCHURES 


Please note the following change in the Individual Blue Edge Dental application and brochure for the Premier Plan:

  • Annual maximum is listed as $1200 and should be $1250.

Going forward, please ensure to communicate the correct annual maximum for the Premier Plan with your clients. 
 

Updated versions of the applications and brochures will be added to the producer portal under the Resources section. You will be notified when these materials are available.

INDIVIDUAL SHORT TERM BLUE APPLICATION SUBMISSION ADDRESS UPDATE


The mailing address for submitting any Short Term Blue applications provided on the application is incorrect. Please use the following address for submitting any future applications:

You will be notified when the application with the correct mailing address is uploaded to the producer portal. Any returned applications should be resent to this mailing address, along with the original envelope, to ensure that the application will have the original requested effective date.

FREE IDENTITY PROTECTION FOR ALL HIGHMARK MEMBERS: CONTIUNUED FOR 2017 


Highmark will continue to offer the no-cost identity protection services to all eligible plan members through AllClear ID
  • If your client has opted in to identity protection services in 2016 and wants to continue coverage for 2017, no action needs to be taken
  • If your client did not enroll for coverage in 2016 but would like to do so for 2017, they will need to enroll through the employer portal no later than Dec. 31, 2016
  • If your client enrolled for 2016, but does not want to continue these services in 2017, they will need to "opt out" using the employer portal by Dec. 31, 2016. IF they do not opt out, coverage will automatically continue for 2017. 
Highmark will not be contacting employees who sign up or renew identity protection services for 2017. Information can be found through the "e-spot" that appears in their member portal account page. 

Any employee who wants to enroll in Credit Monitoring services must take action to enroll; this can be done at any time in 2017. Credit Monitoring services must be renewed annually - AllClear ID is reaching out to those that enrolled in 2016 to provide instructions on how to renew for 2017.
