2017 SMALL GROUP FORMS NOW AVAILABLE
The 2017 Small Group Business Application, Enrollment/Waiver Forms and Member Change Forms are now available on the producer portal under the Resources section. These forms include new CMS disclaimers and foreign language translations on the last two pages.
To comply with the Affordable Care Act, please only use the 2017 forms and destroy all previous versions.
COMING SOON: ONLINE CONTRACT AVAILABILITY
In addition to the new CMS disclaimers, the Small Group Business Application also includes a new Online Contract Availability section on page five. Here, group administrators will have the option to access their group health contracts via a secured Highmark online site. Please note that this option is not yet available - you will be notified when this option is available. Until then, group administrators may skip this section.
Aside from the noted changes listed above, no other revisions have been made to the forms. If you have any questions about the 2017 forms, please contact your Highmark Small Group Client Manager or Sales Executive.
