What makes Aetna Funding AdvantageSM paperwork easier? Having no paperwork at all.

Enhancements to the AFA Small Group sold case process
Paperwork. It’s probably near the top of your to-do list, but the bottom of your want-to-do list. That’s why we’ve been working hard to minimize it for AFA and move towards a paperless environment. Recently, we’ve made big strides in this area – from Aetna Springboard Marketplace®, to Aetna EasyAppsOnline. The changes have made these tasks much simpler and smoother for you and your clients.

Now we’re excited to continue this paperless effort with enhancements to the sold case process, effective November 7th. It’s truly as easy as 1 – 2 – 3.

  1. New Customer Information Request (CIR) form: This form will collect key information to help us prepopulate sold case forms. That’s right – you and your clients won’t have to enter these details on every document. The CIR form saves time, and ensures complete and legible information on the first submission – with an end goal of expediting the set up process.

  2. Sold case emails: Once we receive the completed CIR form, we’ll send emails with sold case forms (which will include prepopulated CIR form details), within 24 hours.  Please note: a separate email will be sent for each form to be sure information is organized and easy to complete. You and the General Agent (if applicable) will be included on all emails and documents sent to your clients. These include:

     - Employer application
     - Stop loss information
     - Required NY HCRA 4399A, 4399B, 4264
     - eList or Aetna Springboard Marketplace census and defined contribution template

For current self-funded or TPA plan sponsors, the following NY HCRA forms will also be required:

     - NY HCRA 4409
     - NY HCRA 4403

  3. EchoSign: This application will be used by you and your clients to complete and sign all the sold case forms.
