2017 PersonalBlue Transitional Plans

Members who are enrolled in one of our PersonalBlue Guaranteed Issue or Medically Underwritten plans may opt to keep their current plan through December 31, 2017. Members do not have to take any action if they would like to renew their current plan. They will be automatically renewed. Members will receive the age band rate increase which will be effective January 1, 2017 and this will be reflected on the execution page that is included in their renewal communication.

Renewal Communications are expected to be mailed to members beginning this week.

• Execution Page Addendum (with premium information)
  
• Rx Creditable Coverage Disclosure Notice
  
• Rx Non-Creditable Coverage Disclosure Notice
