self-funded plan designs.
The following updates apply for new business with effective dates of
12/1/16, and later, and inforce business with new plan years effective on or
after 12/1/2016.

designs available on a stand-alone basis

card changes, including a new specialty drug copay

Inforce groups are being notified about these changes in their Benefit
Change Notification, which is sent out approximately 90 days in advance of
their new plan year.
