Individual short-term health plans are a match for clients who are:
- Under 65
- Recent graduates waiting to land their first job
- Newly hired employees not yet eligible for an employer medical plan
- Missing open enrollment
- Retiring, but not yet eligible for Medicare
Note: Premiums for Short Term Major Medical plans increased as of June 1, 2016. The correct paper application will have the date “1/29/2016” on the bottom right corner of the cover page.

Quick tips for Medicare Supplement applications:
- Use your NPN number to fill out Producer ID—not your BRK number.
- If applicant answers "Yes" to any underwriting question, the application will be denied. (Exception: The daily activities questions can be answered "Yes.")
- Remember to fill in:
—Height and Weight when the application is subject to underwriting.
—Previous Plan ID and Category in Section 4 when the application is subject to the Balanced Budget Act.
—Start Date and Name of the previous plan in Section 8 of the application. This is required to determine if the client had credible coverage. If the application is for individual coverage, indicate the Name and Type of the previous plan (ex: Capital BlueCross Individual Plan).
