Additional Cut-off Deadlines

Capital BlueCross deadline for OFF Exchange is 12-28 for a 1-1. URL MUST have the applications by 10am on 12-28.

HBS Terminated Consumers is 12-31 for a 1-1 for on and off exchange. URL MUST have off exchange applications by 9am on 12-31 as our office closes at noon. For ON Exchange, our Enrollment Center will be closing at 3pm. Download the 2016 Central PA SEP form, and the 2016 Western PA SEP form.
