Certification of Group Size Revised

The Certification of Group Size has been revised. Please begin using this updated version of the form immediately.

Underwriting realizes that producers may already be working with groups for November and December effective dates who have completed the previous version of the form. Thus, they will continue to accept that version for those groups.

All groups with a January effective date and forward must utilize the newest, most current version of the form.
