 New guidance for obtaining member information from UPMC Health Plan


There are two easy ways for you to provide your clients' authorization to obtain member-specific information from UPMC Health Plan.

 

By Phone

Our Health Plan Health Care Concierge team is here to help. The Health Care Concierge can connect with you and your client on a three-way call. During the call, the member may verbally authorize access to their personal information. Verbal permission is necessary every time you need to access your clients' personal information.

 

In Writing

Ask your client to complete and sign the Personal Representation Designation ("PRD") form. Once you have the completed and signed form, send it to your UPMC Health Plan Health Care Concierge via fax (412-454-7829), or mail (UPMC Health Plan, PO Box 2965 Pittsburgh, PA 15230-2965)
  • If we have already received this form, a Health Care Concierge will be able to confirm we have the PRD on file before proceeding. 
  • If you are submitting the form for the first time, and UPMC Health Plan has not yet received/processed it, you will need to represent that you have obtained the appropriate authorization and submitted it to the Health Plan.
 
This PRD form will authorize the Producer's access to member data. If you have any questions about this process, please contact Member Services or your account manager.
 
Leave Information on a Client's Behalf
If you do not have a completed PRD form, you can provide detailed information related to your client's question or issue. The Health Care Concierge cannot release any information, but he or she can confirm that the issue has been submitted and will be investigated.
